NEW PATIENT?

Available to you are our new patient forms. Please download the forms we require for your visit, which include: a medical history, HIPAA privacy act, payment options, financial policy, and privacy practices. By printing them and filling them out in advance, you allow us to attend to your needs more quickly than completing them upon your arrival. If you have any questions regarding these forms, please feel free to contact our office.

IMPORTANT: If you are unable to make your first appointment, please let us know at least 24 hours in advance. Anything under the 24 hour mark could be considered a failed appointment, and may result in not being a patient in our practice. Thank you for your cooperation!

 BROKEN/MISSED APPOINTMENTS

​A specific amount of time is reserved especially for you and we strongly encourage all patients to keep their appointments. If you must change your appointment, we require at least 24 hours notice to avoid a $35/hr cancellation fee (emergencies are an exception). Less than 24 hour notice is a failed appointment and consecutive failed appointments are immediate dismissal.

We offer convenient office hours to accommodate our patient's schedules. Please be aware that not all doctors/hygienists may be available at all times listed.

Monday: 7:00 a.m. to 6:00 p.m.

Tuesday: 7:00 a.m. to 6:00 p.m.

Wednesday: 7:00 a.m. to 7:00 p.m.

Thursday: 7:00 a.m. to 7:00 p.m.

Friday: 8:00 a.m. to 1:00 p.m.

 

 

Payment Options

Payment is expected at the time services are performed. Feel free to contact our office with any questions. For the convenience of our patients, we accept:
